THE GLAD MOWATT LIFE MEMBERSHIP AWARD

It is with great sadness that this past season Evo Parisotto, one of the past President's of GMHA passed away. Evo was not only a past president but also a recipient of the GLAD MOWATT LIFE MEMBERSHIP AWARD, and many other awards.

Evo started his career in 1953 and was selected for the Life Membership Award in 1977. Evo had also been involved with hockey, softball, baseball, and a long time referee in the city for many years. He will be greatly missed by many people.

Sadly, due to Evo’s passing, nominations for this very distinctive award are being accepted. It is reserved to those who have provided very distinctive service to the Association and have served at least five (5) years on the Board of Directors. Life Memberships are limited to not more than five (5) living members at one time.

We would ask therefore that you submit your nomination of a board member, past or present that you feel is deserving of this very prestigious award. Please print the nomination form and deliver your submission to the hockey office.

The criteria pertaining to nominations for this award can be found below. Nominations are to be submitted to the President (president@guelphminorhockey.com) or Recording Secretary (info@guelphminorhockey.com) by hardcopy to the hockey office at least 30 days prior to the next Annual General Meeting. This award is not required to be given out at each Annual General Meeting, but only one per year may be awarded.


The Glad Mowatt Life Membership Award Criteria

  1. Life Memberships are to be awarded for very distinctive service to the Association.
  2. To be reserved for those who have served at least five (5) years on the Board of Directors.
  3. Nominations are to be submitted to the President or Recording Secretary in writing at least thirty (30) days prior to the Annual Meeting.
  4. Each nomination paper must bear the signatures of at least five (5) members of the Board of Directors signifying approval for each proposed member.
  5. Life Memberships shall be limited to not more than five (5) living members.
  6. Not more than one (1) award shall be made each year up to the maximum of five living members.
  7. Life Members shall have the privilege of the Board of Directors of Association, and shall be eligible to serve on all committees if and when appointed to do so. They shall be permitted a vote in such a capacity.
  8. The award study and screening committee shall consist of one member as appointed by the Board of Directors and the last two available recipients. Presentation of this award to be by a life member.
