2023-24 Member Registration (Voting Purposes)
The purpose of this form is to become a registered member with GMHA to have a vote at the GMHA AGM. Families with one child are allowed one vote combined. Families with two or more children are allowed two votes combined. Volunteers such as board members, coaches, trainers, managers, and convenors are also eligible for one vote. One individual cannot receive multiple votes for being in multiple roles (ie: A parent of a player who volunteers as a coach still only receives one vote.).

Participant Information

For every child you have enrolled in Guelph Minor Hockey, please fill out their first name, last name & date of birth.

I/My participating child hereby agree to abide by and support the current Ontario Minor Hockey Association Code of Conduct, and rules, regulations and decisions of the OMHA, OHF, HC and GMHA. The OMHA Code of Conduct identifies the standard of behaviour, which is expected of all OMHA members and participants. As a member of the OMHA, for the purpose of this policy participants shall include all GMHA players, parents, guardians, coaches, officials, volunteers, directors, officers, committee members, convenors, team managers, trainers, administrators and employees.

GMHA is committed to providing an environment in which all individuals are treated with respect.


Members and participants of the GMHA shall conduct themselves at all times in a manner consistent with the values of the OMHA and GMHA, which includes fairness, integrity and mutual respect. During the course of all OMHA and GMHA activities and events, members shall avoid behaviour, which brings OMHA, GMHA or the sport of hockey into disrepute, including but not limited to the use of alcohol, cannabis and non-medical drugs.


GMHA members and participants shall at all times adhere to OMHA operational policies and procedures, rules and regulations governing OHF events and activities, to rules and regulations governing any competitions in which participants play on behalf of the OMHA and to the GMHA By-Laws, rules of operation, policies and procedures.


I, as a member and participant of GMHA, shall not engage in any activities or behaviours which interferes with a competition or with any player or team’s preparation for a competition or which endanger the safety of others.


I, as a member of GMHA, shall refrain from comments or behaviours, which are disrespectful, offensive, abusive, racist, or sexist. In particular, behaviour, which constitutes harassment or abuse, will not be tolerated and will be dealt with under OMHA’s Harassment Policy and GMHA’s By-laws.


I, as a member or participant, acknowledge that from time to time, I/my child will be travelling from place to place, sleeping away from home, and eating away from home. I/my child may reserve the right to take action against a wrongdoer but, subject to that, I release the OMHA and GMHA, including all members, coaches, managers, and directors, from any responsibility.


I hereby understand and appreciate that participation as a hockey player carries risks to me or my participating child of serious injury, including permanent disability, paralysis, or death. I/my participating child voluntarily and knowingly acknowledge, accept and assume these risks.


Failure to comply with this Member Agreement may result in disciplinary action in accordance with the Discipline Policy of the OMHA and GMHA By-laws. Such action may result in the member losing the privileges that come with membership in GMHA and OMHA, including the opportunity to participate in GMHA and OMHA activities and events, both present and future.


I the participant/parent have become subject to the rules, regulations and decisions of the OMHA, OHF, HC and GMHA and its Board of Directors and agree to abide by such rules, regulations and decisions of the OMHA, OHF, HC and GMHA. I am aware that these rules and regulations are available to me through my local association and certify that the provided information is true.


The above named Participant and Parent/Guardian (if applicable) has/have reviewed the OHF Concussion Code of Conduct and the appropriate Concussion Awareness Resources and commit to operating within the parameters of the OHF Concussion Code of Conduct under the role which the Participant and Parent/Guardian (if applicable) has/have registered with the OHF.* All information can be reviewed here:

"The Ontario Government has enacted Rowan’s Law (Concussion Safety), 2018, S.O. 2018, c. 1 (“Act”). Ontario Regulation 161/19, the Act requires all sport organizations as defined in the Regulation (“Sports Organization”), which includes the Ontario Hockey Federation (“OHF”), to have a Concussion Code of Conduct. This Concussion Code of Conduct must require participants, as set out in the Act, to review the Ontario Government’s issued Concussion Awareness Resources on an annual basis. 
A participant is subject to a Concussion Code of Conduct for each Sports Organization a participant registers with.
The OHF Concussion Code of Conduct and the appropriate Concussion Awareness Resources must be reviewed before you can register/participate in the OHF.

I authorize GMHA and/or their respective staff, associates, assistants, or subcontractors to use photograph(s), video and other media images provided for any GMHA promotional, educational or other pertinent uses. I also agree to GMHA using any images of the named minor(s) gathered by photographers, and or videographers supplying images to GMHA. These images may include, but are not exclusive to, club, coach, athlete and/or parent, and/or volunteer submissions by various member associations of GMHA. I authorize GMHA to permit the use and display of photographs and/or recordings of the named minors in any GMHA publication, multimedia production, including video and web usage, display, or advertisement. I agree that GMHA may use name, likeness, or information supplied by the undersigned. The undersigned releases and forever discharges GMHA and their respective officers and employees from any and all claims and demands arising out of or in connection with the use of said photographs / recordings, including but not limited to, any claims for invasion of privacy or defamation.
