Ensure you
complete all mandatory questions marked with an asterisk. Additionally, prepare
a one-page Coaching Philosophy/Coaching Motivation, Hockey Resume, and an
age-appropriate 50-minute practice plan in PDF format. The application form
will prompt you to attach these documents with your name (e.g., Connor McDavid
- Practice Plan). You will receive an email confirmation upon successful
submission.
